Schedule ¶¹ÄÌÊÓƵ Student Vaccination (Pfizer), 12+ - October 12th
A Pfizer vaccination distribution has been scheduled for Tuesday, October 12th from 3pm - 4:45pm at Newburgh Free Academy Main Campus - enter lower lobby. The Pfizer vaccine has been authorized for people 12 years of age and up. Students and their families are welcome to participate in this immunization. This distribution is facilitated by Middletown Medical.
Tuesday, October 12th from 3pm - 3:45pm (Second dose TBD):
Newburgh Free Academy Main Campus - Lower level (201 Fullerton Ave., Newburgh)
Both the student being vaccinated and the accompanying parent/guardian need to bring appropriate identification (birth certificate, social security card, parent ID, passport, consulate ID, driver's license, municipal ID, etc.) in order to receive the vaccine. Please bring insurance information with you if you have it. If you do not have insurance, that is okay, you will still be able to get the vaccination.
Scholars who are interested can share this form with family members if they would like to receive the vaccine together.
The clinics have been made possible by several school nursing and teacher volunteers, as well as current and alumni members of the Certified Nursing Assistant Program. We are so thankful for their generosity and expertise! These distributions are in collaboration with Middletown Medical.
